10.7 Class Project Option
Advanced Directive & Planning – Five Wishes Discussion With Family
You are already required to complete the Five Wishes document for this course (available for $5 from the Five Wishes website store: Start-Page – Five Wishes).
This project option allows you to extend end of life planning and clarity to loved ones – IF (and only if) they are receptive and interested in doing so. By completing your own Five Wishes (already required for the course) you are relieving loved ones of making excruciating decisions without guidance. By having this conversation with loved ones and encouraging them to do the same, you are increasing the likelihood that their care wishes for themselves will be known and respected and you may even be alleviating distress and anxiety of your own later if you are responsible for making those decisions.
The same link above includes an option ($20) to purchase the Five Wishes “Starter Pack” – which provides 5 copies of the Five Wishes document and a “Conversation Guide” to facilitate these discussions with loved ones. For this assignment, you would identify at least 2 other people (but no more than 5) to initiate a discussion with regarding end of life planning and have them complete the Five Wishes document. To complete the assignment, you would need to submit 2 items: 1) screenshots or pics of the Five Wishes cover page completed by loved ones/family members and 2) a 2000 word essay (APA format) describing the process of and reflections about engaging in this activity with loved ones.
